Providing Hope and Independence
Referring yourself or a family member to Cleveland Sight Center opens the door to expert services that enhance personal safety, preserve independence, and reduce the fears that often result from vision loss. Those who retain partial vision can learn skills and the use of adaptive visual aids to achieve optimal comfort and effectiveness. How to refer someone
For services to those younger than twenty-three years of age, we recommend a parent or caregiver call 216-791-8118 and ask for “registration.” An experienced intake worker will take the information required to begin service. You can expedite our services, if you ask your eye doctor to fax or email a Children’s Eye Report to our attention.

For services to those over the age of twenty-three, simply call 216-791-8118 and ask for “registration.” We can begin services without a completed Adult Eye Report, but the medical information helps us focus on your particular conditions. Please ask your eye care professional to visit our website, click on Adult Eye Report, and send us the completed form.
