Services for Children, Teens and Young Adults
If you are 22 years of age or younger, or are the parent or guardian of someone 22 years of age or younger, Cleveland Sight Center has special services for you. Learn more about services for children, teens and young adults.
Parents and Guardians
Please note that the EYE REPORT form on this website is for individuals ages 23 and over, and is NOT used for children, teens and young adults. If you are the parent or guardian of a child, teen or young adult, contact Lorie Marsalis in Children's Services for the appropriate case manager by calling (216) 791-8118, x286. Services for Adults
If you are between the ages of 23 and 55, you fall into the category of working-age adult. Learn more about our services for working-age adults.

Choosing the Right Services
Many services, such as our recreational programs and social activities, overlap age groups and are available for every age. Learn more about our services for every age.
Registration for Services
For individuals ages 23 and over, after your initial call or
referral, and upon completing
the registration process, your case manager will begin
working with you and will continue to be
your personal advocate for care. Your case manager will be your primary contact at Cleveland Sight Center; however, you must first be
registered in order for us to begin to assist you. For parents and
guardians of children, teens and young adults ages 22 and under, initial contact
will be with your Children’s Services
case manager.
To complete the registration
process, we will need
signed permission allowing your eye
doctor to release your Eye Report to us, and you will need to have had an eye
exam within the last 12 months. You or your doctor can download an eye report form. Upon receipt of this information, your
case manager will begin assisting you. Your case
manager will follow your progress,
and will always be available to
ensure you are receiving the services
you need. In addition to your case
manager, Cleveland Sight
Center staff, volunteers, and other clients are always
ready to assist you.
